We provide seniors with digital devices to monitor their progress between office visits. Our clinical staff supports these patients through behavioral interventions to help patients build healthy habits and collaborate with the patient’s doctors. We’re looking for a professional and experienced Customer Service Executive to join our team full-time.

The day-to-day responsibilities include being on our service frontline, responding to online inquiries, enrolling and engaging new members, and addressing questions about our program.

You will be responsible for responding to inbound and outbound customer inquiries. You will use research, empathy and critical thinking to help provide issue resolution. You’ll be an integral part of our team and culture, and will collaborate and help the clinical and sales teams with administrative tasks and contribute to always improving our services.

This is a remote position available to anyone in the California area. On-target salary is $75k/yr, $40k base + $35k commissions/bonuses.

Must-have:

  • 5+ years of customer service/sales experience with positive customer feedback statistics
  • Ability to multitask between sales, customer service, and marketing and support members across multiple communication channels, i.e. phone, email, chat, etc.
  • Comfortable using online tools such as Google Docs/Sheets, CRM, Asana/Trello, Google Meet, Zoom • Motivated to help improve the health of our members

We're looking for a certified medical assistant to join our clinical team full-time. Work setting will flex between our client's offices (physician-run clinics) and your own home office.

Compensation: $40,000 to $42,000 annually

Employment type: Full time

The ideal candidate:

  • Has experience in primary care, family practice, geriatrics, and disease management.
  • 4+ years of experience
  • Bonus points for previous customer service, public relations, or sales and marketing experience.
  • Is comfortable with digital technology (including tools like MS Office, Google, various EMRs, etc.).
  • Is familiar with Medicare, chronic conditions management (CCM), remote patient monitoring (RPM), and quality measures (HEDIS).
  • Is familiar with preventive physical examinations (consistent with HCC & Annual Wellness Visit criteria).
  • Collaborates with in-house clinical team team to engage and follow up with patients who are being monitored between visits using our wireless monitoring devices and mobile app.
  • Collaborates with patients’ clinical care team and specialists.
  • Follows up with patients on a regular basis via phone and messaging apps.
  • Is compassionate and good at listening to patient concerns.
  • Documents patient interactions using our online portal.
  • Communicates inter-professionally as needed for continuity of care.
  • Reviews patient charts to ensure all relevant problems and diagnoses have been accurately documented.
  • Is organized, efficient, and adaptable: able to carry out a variety of administrative and clinical duties.
  • Has the ability to think quickly, solve problems and bring professionalism to all situations.

We're looking for a licensed vocational nurse (LVN) to join our clinical team full-time. This position is in the Bay Area. Work setting will flex between our client's offices (physician-run clinics) and your own home office.

Compensation: $50,000 to $56,000 annually

Compensation: $70,000 to $120,000 annually

  • 5+ years of industry experience building web and backend applications Experience developing software in Python, Django, PostgreSQL, Javascript Experience developing interactive visualizations using Javascript and/or frameworks i.e. React.js, Redux, or Vue.js
